Insurance

• If you start vision therapy with our office, and your insurance will reimburse you for the therapy, we can provide you with paperwork to submit to your insurance for reimbursement.  You will receive monthly statements which reflect dates of service and cost for the month, which you can submit to your insurance company.  Please notify our office if you need these forms
• We do not accept insurance, file claims, or accept payment from insurance.
• We do not accept Medicare or Medicaid.
• There are three codes we use for our vision assessment: 92002, 96112, 92060.  We encourage you to check with your insurance company ahead of time to see if they will cover these codes.
• There are four codes that we use for vision therapy: 92065, 97110, 97112, 97530.  We encourage you to check with your insurance company ahead of time to see if they will cover these codes..
